5. Complete the application form
Key points to note are:
1. If the region is set to "provincial-level international," then only "provincial-level international" should be entered in the "region/setting area" field. A document outlining the necessity of setting up and using a provincial-level amateur radio station must also be submitted (refer to the template provided for formatting).
2. Signature section for applicant: Please sign your name and have your guardian sign (write the date below the signature, which should be the application date).
3. Required documents: A scanned copy of my ID card, a scanned copy of the guardian's ID card, proof of guardianship (household registration book, birth certificate), a photo of the front of the operating license, photos of the amateur radio station's body and antenna, and an amateur radio station approval certificate (if available).
All of the above materials can be used in electronic format. (If you need to sign, you can print them out and then digitally add a transparent overlay for signing, which can then be uploaded. All relevant application documents should be formatted within a Word document.) Finally, combine everything into one PPT file, naming the file "XXX Amateur Radio Station Application Form".
Finally, upload the same PDF file to the last eight locations and click "Submit".
