1. For new participants taking the A-class amateur radio operator skills exam, the email subject line should be: "My Name - A-Class Amateur Radio Operator Skills Exam Registration," such as: "Zhang San - A-Class Amateur Radio Operator Skills Exam Registration."
2. For candidates taking a make-up exam, the email subject line should be: "Application for Make-up Exam - [Your Name] Class A Amateur Radio Operating Skills Examination". For example: "Zhang San Application for Make-up Exam - Class A Amateur Radio Operating Skills Examination + Previous Exam Date"
Two. Attachments (All relevant materials must be submitted as attachments)
Please note: Incomplete or incorrectly formatted applications will be considered invalid and will not pass the screening process, preventing you from taking the exam.
Email attachment 1: Registration form (electronic document)
(1) Generate an electronic registration form document using the designated registration system.
● Log in to the website "Guangdong Provincial Radio Station Information Filling System" (URL:http://www.gdradio.gd.gov.cn:18084/RsApply/), in the "Amateur Radio Operator Certificate Application" section, please check the "Application" box in the upper right corner (default application). Fill out the application information as required; if incomplete, the system will not be able to export it.
• After completing the form, click the "Export" button. The system will generate a registration information file, and be sure to save it.
(ii) Create your own electronic registration form document.
1. Personal information should be entered in a TXT document, and must strictly adhere to the template format. Each data item should occupy a separate line, and the order of the data cannot be reversed. The content to the right of each equal sign should be supplemented with the applicant's personal information. If there is no relevant information, it can be omitted, but data items cannot be arbitrarily deleted. Empty lines are not allowed, and there must be a space on both sides of the equal sign. The content entered cannot contain spaces. The template is as follows (red text indicates sample content):
Please note:
1. All dates must be entered in the format specified above, i.e., yyyy.mm.dd.
2. Original call sign (leave blank, do not fill in "blank").
3, Date of exam completion (to be filled in by the examination institution), do not fill this field; leave it blank. Do not enter "Leave Blank" or include the text "(To be filled in by the examination institution)".
4, Examination passing location (to be filled in by the examination institution), do not fill this out, leave it blank, and do not write "leave blank". Do not delete the text "(to be filled in by the examination institution)".
5. "Application operation category" field, enter "A", without quotation marks;
6. "Shipping Address Province/Region/City" – Please specify the province, autonomous region, municipality, or special administrative region where the applicant resides, for example: Guangdong Province, Guangxi Zhuang Autonomous Region, Shanghai Municipality, Hong Kong Special Administrative Region;
7. "Shipping Address - City, County": Please enter the name of the prefecture-level city where the applicant resides (Note: Guangdong has 21 prefecture-level cities; please ensure you do not list a county-level city instead; this item does not need to be repeated for XX province).
8. "Shipping Address: Street and Building Number" - Please fill in the county-level city, district, or town where the applicant resides. The address should be specified to the street and building number level for urban areas, and to the township (town), village group, etc., for rural areas (this item does not need to be repeated for XX Province, XX City).
9. "Postal Code" field must be filled with 6 digits.
10. "Contact Phone" field, must be filled with the mobile phone number used by the applicant, which consists of 11 digits and cannot include a landline number;
11. "Email" field: The email address must contain "@".
12. "QQ" section, please fill in your personal QQ number (a purely numeric value), if you do not have a QQ number, apply for one temporarily for the license application.
13. "Notes" field: If no special instructions are provided, this field should be left blank and not filled with "Blank".
14. The shipping address, mobile phone number, email address, and QQ account must be your own, not those of others or a group.
15. Requirements for the name of the TXT document file: The file name must be modified according to the format "Registration Form - Personal Name [Personal ID Number].txt", otherwise the system will not be able to process it. For example: Registration Form - Zhang San[440101201503201213].txt
(ii) Appendix II: Electronic version of identification photo document
• Applicants must submit one electronic copy of a white background photo, taken within the past six months, (photos taken in everyday life are not allowed), in JPG format. The image must be 480 x 640 pixels; otherwise, the system will not be able to accept it.
• The photo files should be named "Applicant ID + Applicant Name.jpg", for example: 440101201503201213 Zhang San.jpg.
(iii) Appendix Three: Electronic version of identity verification documents
1. Applicants must submit scanned copies of their own ID card, with both the front and back sides clearly visible. The images should be in JPEG format, with a resolution between 1024×768 pixels and 3000×2000 pixels. The file name should be: "Applicant's Name and Document Type". For example: "Zhang San ID Front.jpg".
2. For applicants from Hong Kong and Macao, the required identification documents are scans of both sides of their "Hong Kong and Macau Residents' Travel Permit," with each side scanned separately; in JPG format, with a resolution no lower than 1024×768 pixels and no higher than 3000×2000 pixels; the file name must consist of the applicant's name and the document name. For example: "Zhang San Travel Permit - Front.jpg".
